Summit High School Enrollment Check Sheet
Dear Student and Parent/ Guardian:

Welcome to Summit High School! We at Summit feel our school is the best place in the world for secondary education. In order to make your enrollment a pleasant and easy experience, please follow the guidelines below and use them as a checklist. Thank you.


  1. Pick up an enrollment packet from the Counseling Office.
  2. If your child speaks another language other than English or is bilingual, he/she will need to take an assessment test after meeting with the counselor. The Assessment center is located in the Central Enrollment Center, 9548 Citrus Ave., Building A, Room #A24, (909) 357-5000 Ext. 7439.
  3. If your child has Special Education Placement, please call Irene Gomez at 357-5000 Ext. 7332 or go to Window 3 at the Central Enrollment Center, 9548 Citrus Ave. Building A. Once you have met with Mrs. Gomez, please call 357-5950 to make an enrollment appointment.
  4. See the Summit nurse, located on the bottom floor of the administrations building, to have your child's shot records verified. Immunizations are required by law. A parent/guardian must accompany the student.
  5. Call the counseling office at 357-5950, ext. 3035, and speak to the secretary for an appointment to enroll your student(s). If all paperwork is in order, this will be student's first day in school. Due to limited seating, we ask that only the parent/ guardian and student attend the enrollment appointment.

    A PARENT OR GUARDIAN MUST ACCOMPANY STUDENT.
  6. Once you have been given an appointment with a counselor, please be sure to bring the following materials:
  • Check-out grades from the last school of attendance (after 15 days).
  • Transcript (may be unofficial) from the last school of attendance. Please feel free to have the school fax transcripts to Summit High School: (909) 357-5698. "Attention Counseling Secretary".
  • Proof of Residency in parent/guardian's name- Rental Agreement; Escrow Instructions; Utility Bill; home telephone bill. We do not accept cable or cell phone bills. All utilities must be dated within the last 30 days only- If the bills are not in the parent/guardian's name the person with whom the family lives with must be present at the time of the appointment along with the parent/guardian, to fill out a Declaration of Parental residency Form and provide the following :
  • Parent's ID and Resident's ID and matching utility bill are required. The child(ren) must be living with the legal parent or legal court appointed guardian in order to enroll in the Fontana Unified School District. Court documents must be present at the time of the enrollment appointment. If there are no court papers, the guardian will need to sign a Caregiver Form. (Education Code 48200).
  • Immunization records if you have not already seen the nurse. The student will not be enrolled without an immunization record.
  • From out of state or country - Birth Certificate required.

PLEASE NOTE- If the student's last school was Alternative or Continuation, you will need to call (909) 357-5000, ext. 7275. If your student has been expelled or recommended for expulsion from any school district within the past two years and has been reinstated, you must first register with our Child Welfare and Attendance Office located at 9458 Citrus Ave., Bldg. A27, (909) 357-5000, ext. 7089 or ext. 7096, before proceeding with an enrollment appointment at the school site. Please be sure to bring any expulsion paperwork to CWA.

IF YOU ARE MORE THAN 10 MINUTES LATE TO YOUR ENROLLMENT APPOINTMENT, WE WILL NEED TO RESCHEDULE YOUR APPOINTMENT!
